If your child requires medication in school the Medication Consent Form (below) must be completed, signed and returned to the school.

Please note: all medication must have a prescription sticker on the side of the box/bottle with the child’s name on and with directions of how/when to take their medication. 

Students must then bring both the form and medication to be kept in the Medical Office and administered by the school's Medical Officer Ms Barnard; to contact her please email ellie.barnard@furzeplatt.net
